- make the eligible purchase

- fill out and mail in the rebate form.
Within four weeks of approving your claim, Hunter Douglas will mail you the appropriate rebate in the form of a prepaid rewards card. Even though funds do not expire, the rebate is subject to applicable law. A $2.00 monthly fee will be assessed against the card balance six months after card issuance and each month thereafter.
